With an immediate implant, the implant is placed in the same session in which a tooth that cannot be saved is removed. This can spare you an additional procedure and waiting time – provided the bone, gums and any infection allow it. At Dr. Coşkun's practice in Istanbul, this decision is made on the basis of a 3D scan and a thorough examination, not promised in advance.
What Is Immediate Implant Placement?
Traditionally, the socket is left to heal for several weeks to months after an extraction before the implant is placed. With immediate placement, both steps are combined: the tooth is removed as gently as possible and the implant is anchored directly in the fresh socket.
This is different from immediate loading, which describes whether the implant receives a temporary restoration straight away. An immediate implant can heal protected under the gum or – if it is stable enough – receive a temporary crown on the same day. The permanent crown follows only after healing.

Who Is a Candidate?
Whether an immediate implant makes sense is decided case by case. Key criteria are:
- Sufficient bone: There must be enough bone around and below the socket for the implant to be firm from the start (primary stability).
- Intact bone walls: Especially the thin outer bone wall in the front of the mouth should be preserved.
- No acute infection: Acute pus or extensive inflammation at the root tip often favours a staged approach.
- Healthy gums: Any gum disease should be treated first.
- General factors: Smoking, poorly controlled diabetes or certain medications can affect healing and are discussed beforehand.
If one of these conditions is not met, a delayed approach is often the more reliable choice – for example preserving the socket with bone substitute material and placing the implant after a few months.
The Treatment Step by Step
- Diagnosis and planning: Clinical examination and usually a 3D scan (CBCT). The position, length and diameter of the implant are planned digitally.
- Gentle extraction: Under local anaesthesia, the tooth is removed while protecting the bone walls as far as possible, and the socket is cleaned thoroughly.
- Implant placement: The implant is anchored in the socket. Gaps between the implant and the bone wall can be filled with bone substitute material.
- Temporary: Depending on stability, you receive a temporary crown or a removable interim solution so that no visible gap remains.
- Healing: Usually 3 to 6 months, depending on the jaw, bone quality and any bone grafting.
- Permanent crown: After healing, the final crown is made and fitted – for patients from abroad typically on the second trip.
Possible Advantages
- One procedure fewer: Extraction and implant placement take place in one session.
- Shorter overall treatment time: The separate socket healing phase is not needed.
- Aesthetics in the front: With a temporary, there is no visible gap, and the gum contour can be supported.
- Fewer trips: For patients travelling from abroad, this can simplify planning.
Limitations and Risks
Immediate placement is technically more demanding than placing an implant in healed bone. In the front of the mouth, the gum may recede slightly over time. As with any implant procedure, swelling, bruising, infection or failure of the implant to integrate are possible. That is why an immediate implant is only recommended when the starting situation supports it – a particular outcome cannot be guaranteed.
After the Procedure
- Cool your cheek on and off for the first few hours.
- Wait until the anaesthesia has worn off before eating, and chew on the other side at first.
- Do not bite hard foods with a temporary crown – at this stage it is mainly there for appearance.
- Avoid smoking if possible, as it can impair healing.
- Keep your follow-up appointments and follow the oral hygiene instructions.

Is an Immediate Implant Right for You?
Whether immediate placement is possible can often be assessed in advance from a recent X-ray or 3D scan. Dr. Coşkun makes the final decision after examining you in Istanbul – and will tell you plainly if a staged approach is the better choice.
Frequently Asked Questions
Will I get fixed teeth straight away with an immediate implant?
A temporary crown on the same day is often possible if the implant is stable enough. It is not meant for full chewing load. The permanent crown follows after healing, usually after 3 to 6 months.
Is an immediate implant less reliable than a delayed one?
In carefully selected cases, studies show survival rates comparable to delayed placement. The right indication is what matters: if bone is missing or there is an acute infection, a staged approach is usually more reliable.
Does immediate implant placement hurt?
The procedure is carried out under local anaesthesia. Swelling and a feeling of pressure are common in the days afterwards and can usually be managed well with standard painkillers.
Can I fly home right after an immediate implant?
Flying home is usually possible after a few days and a check-up. If bone grafting was done in the upper back jaw at the same time, a slightly longer interval may be advisable. We discuss this individually.
How can I find out in advance whether an immediate implant is possible for me?
Send us a recent X-ray or 3D scan. Dr. Coşkun will give you an initial assessment; the final decision is made after the examination in Istanbul.
